Two-time Super Bowl champion Hines Ward added another trophy to his collection Tuesday evening, as the Pittsburgh Steelers wide receiver was named the winner of 'Dancing With the Stars' season 12.

Karina Smirnoff ended Monday night's episode of 'Dancing With the Stars' season 12 on a bit of a low note, when the 33-year-old beauty fell in the middle of the ballroom floor.

The tumble happened near the beginning of her Paso Doble routine, when she accidentally slipped on partner Ralph Macchio's coat.

Luckily, the pair recover nicely -- enough to receive a score of 24 out of 30 from the judges. Even better, it wasn't even the lowest score of the night.

Petra Nemcova Eliminated From ‘Dancing With the Stars’; Pia Toscano to Perform Next Week

Petra Nemcova is officially out of style, as the 31-year-old model was eliminated tonight from 'Dancing With the Stars' season 12.

The results weren't too surprising; Nemcova and partner Dmitry Chaplin's quickstep tied for the lowest judges' score on Monday (22).

Still, Nemcova was unable to fight back tears following her elimination, and later referred to her experience on 'Dancing' as "life-changing."

Poor Kirstie Alley. Just one week after she fell on the 'Dancing With the Stars' ballroom floor, the Emmy-winning actress suffered another unfortunate mishap.

In this case, it was her shoe that fell (off). Midway through her routine last night, a noticeably embarrassed Alley remained seated on the floor for a few moments while she put herself back together again.

Luckily, the performance hiccup didn't deter her too much. As was the case last week, Alley got right back on her feet and finished her performance with grace and style -- not to mention a solid score of 22.

'Dancing With the Stars' season 12 premiered tonight, and while it lacked the headline power of Kate Gosselin and Bristol Palin, there were plenty of memorable moments that will have everyone talking at the post-show watercooler. Here are the four that stood out the most:

1.) A Star Is Reborn
Ralph Macchio may not be the most famous member of the season 12 cast, but the 'Karate Kid' star definitely made a name for himself tonight with a show-stopping foxtrot. The routine earned Macchio and partner Karina Smirnoff the highest score of the night (24), and set the bar high for the rest of the season. After tonight, Macchio moved one quick step closer to the Mirror Ball Trophy.
